The Total Estimated Cost for Testing Services (Evaluations) includes records review, interview(s), administration & scoring of relevant tests, interpretation of all results, written report, and discussion of the results.  Additional services can be requested and may require an additional cost (example: attendance at meetings, extra document preparation, etc.). Digital testing is a fee paid by the patient/guarantor to use a subscribed professional digital testing system to administer assessments online quickly and efficiently, generate accurate scores, and produce individual reports for each test administered. Digital testing services are performed in a secure, private website environment that helps professionals streamline their assessment workflow. Digital testing fees are mainly used to pay for this service. For instance, the price of individual digital tests can vary from $3.65 to $40.00. The number of digital tests will be determined by the psychologist. Similarly, the Total Estimated Cost for Treatment (number of prescribed hours/sessions) may vary. If these additional services are requested, a new cost estimate will be provided.

 

Although we make reasonable efforts to verify patients’ insurance benefits, each patient is responsible for understanding their own insurance benefits. Note that the authorization and quote of benefits we receive from an insurance company is NOT a guarantee of payment from the patient’s carrier. If the actual benefit is different, the guarantor will be billed the remaining balance or refunded the credit. Please understand that patients are ultimately financially responsible for any balance not covered by your insurance plan. Medical records, including an evaluation report, will be provided once the patient’s financial responsibility has been paid.
